HGV Paperwork
There are a few documents that you need to proceed with your LGV application. Some applies to just applicants of a C licence and some applies to C+E applicants.
HGV Class C (Class 2) Applicants
You will need to get form D2 from your local Post Office as well as a D4 form.
The D2 form is an application form and you need to fill this in.
The D4 is a medical report form and your doctor will need this when you go for your medical.
Both forms, when completed, need to be sent along with any fees payable (refer to the DVLA's INS115 form for fees) and your existing licence to the DVLA
They will need both the plastic card and paper counterpart if you have a photocard licence or just the paper licence if you do not have a photocard licence. If you fall into the latter category, you will need to send a photo so a photocard licence can be made for you.
Once you have your provisional licence, you can learn to drive.
You will need to take a theory test and a hazard perception test prior to taking the practical test.
Once you have passed your practical test, send your provisional licence to the DVLA along with your test pass certificate (D10V) with any fees payable to the DVLA.
HGV Class C+E (Class 1) Applicants
